Care Navigator 1.0

The Nottingham Care Navigator (NCN) currently serves as a resource for General Practitioners and Consultants involved in the care of Nottingham residents requiring hospital services. It facilitates access to appropriate clinical advice and support, aiming to connect patients with the right service at the first point of contact.

The platform’s services and information have been developed with two primary objectives. Firstly, to enable GPs to directly consult with specialists, thereby streamlining the referral process. This allows for the reservation of urgent outpatient appointments when necessary, helping to potentially prevent unnecessary hospital admissions. Secondly, it supports hospital-based Consultants in coordinating timely follow-up care within primary care settings.
